A Geriatric Care Manager (GCM) is a resource expert who specializes in assisting with senior care issues as well as long term care arrangements for older adults and their families. Geriatric Care Management personnel are trained and experienced in the assessment, coordination, monitoring and direct delivery of services to help clients remain in the least restrictive environment.
