Private Services

A Caring Hand, our private care subsidiary, provides home care aides, live-ins, personal care attendants, homemakers and companions. Services include bathing, dressing, transferring, housekeeping and companionship for an out-of-pocket fee.

VNHSC’s geriatric care management program provides an assessment along with coordination, monitoring and direct delivery of services to help clients remain in the least restrictive environment.

A Caring Hand

A Caring Hand provides you and your loved ones with another option: to remain in the comfort of your own home while receiving the one-on-one care you deserve in your time of need. We provide an affordable alternative long-term solution when you need it the most. Our staff can assist with all daily routines in the privacy and comfort of your own home. A Caring Hand is an option we all deserve. Our caring staff have been carefully selected and thoroughly trained to meet your specific needs. Each of our staff is carefully screened, bonded and insured for your peace of mind. Most importantly, in addition to their skills as personal care aides, homemakers and companions, our staff has been noted for their trustworthiness, caring and compassion.

An affiliate of Visiting Nurse & Health Services of Connecticut, A Caring Hand works closely with homecare nurses, therapists and other community based providers. This collaboration ensures you and your family that a team of health care professionals is actively involved and coordinating services if needed.

Companions

Companions provide the support and supervision that will enable you or your loved one to remain independent, while getting the assistance they need to remain safe and comfortable in their own home. Services include:

  • Assistance with medication reminders
  • Companionship, socialization and reading
  • Meal preparations and errands
  • Transportation to physicians, accompaniment to shopping or recreational activities
Specialty Companions are available for:
  • Alzheimer’s Care Companions are skilled at communicating with people diagnosed with Alzheimer’s. They are also trained in recreational activities, music and touch therapies, and all components to care for clients with Alzheimer’s.
  • Hospice Care Companions are skilled at caring for terminally ill clients. They bring compassion and the ability to attend to your loved one’s needs during their final days.
Homemakers

A homemaker can help you maintain your home in the way you want, ensuring a clean and safe environment. Services include:

  • Errands
  • Grocery shopping
  • Housekeeping
  • Laundry
  • Meal planning and preparation
  • Medication reminders
Personal Care Assistant

Personal Care Assistance is available for those who are in need of assistance with personal hygiene and more. Services include:

  • Assistance with Ambulation
  • Bathing
  • Dental Hygiene Assistance
  • Dressing
  • Medication reminders
  • Shampooing and drying hair
  • Toileting

Personal Care Assistants respect one’s dignity, while helping to maintain independence in the home.

Live In Care

A Live In can provide invaluable peace of mind for a family member or other caregiver while they work, shop or on vacation, ensuring both security and socialization for their loved one. Whether it is to share a game, read, serve a meal or just listen, a trustworthy companion from A Caring Hand can be there. Services include:

Geriatric Care Management

VNHSC’s geriatric care management program provides an assessment along with coordination, monitoring and direct delivery of services to help clients remain in the least restrictive environment.

  • Are you overwhelmed by the needs of an aging parent?
  • Is concern for your parent’s health taking you away from your family?
  • Are your parents safe at home?

If so, you and your family may benefit from our geriatric care management program. A geriatric care manager is a resource expert who specializes in assisting with senior care issues as well as long term care arrangements for older adults and their families.

Geriatric care management personnel are trained and experienced in the assessment, coordination, monitoring and direct delivery of services to help clients remain in the least restrictive environment.

A geriatric care manager will provide assessment of the client. Together, the client, family and the geriatric care manager are involved in the plan of care. Needs may change rapidly and through close monitoring the geriatric care manager can make the necessary and immediate changes to help clients living at home remain as safe and independent as possible.

A Geriatric Care Manager will:

  • Provide a comprehensive physical and social needs assessment
  • Develop, coordinate and modify a care plan to assist with day-to-day activities
  • Arrange and monitor in-home help or services
  • Act as a liaison to families at a distance
  • Provide a link to community resources available to the client
  • Review financial, legal or medical issues and refer to the appropriate resources
  • Assist with town, state and federal applications
  • Provide counseling and crisis intervention to address emergency situations
  • Assist with alternative living arrangements when home is no longer an option
  • Provide ongoing support and counseling to the clients’ families
